Medication Authority Form

It is essential that we know about any medication your child may need to take. Please fill out the form to authorise your child to take medication whilst they are at school

Extended Leave / Travel Form

Any leave or travel within Australia or overseas lasting more than 3 days requires for an extended leave request to be submitted. Please complete this form to have your leave reviewed by the principal. You must provide a copy of your flight tickets and/or itinerary for your request.
